AMYLOID ARTHROPATHY ASSOCIATED LIGHT CHAIN MYELOMA: A CASE REPORT

Amyloidosis consists of a heteregeneous group of diseases characterized by deposition of a proteinaceous material in various tissues and organs and occurs in 10-15% of patients with multiple myeloma, but amyloid arthropathy is infrequent in these patients (<5%). A 43 year-old female patient had bilateral pain and stiffness in her joints-, worsening with time. Joint symptoms and findings clinically simulated to those of rheumatoid arthritis. She also suffered from carpal tunnel syndrome. Marked bone marrow infiltration by plasma cells, electrophoretic studies of serum and urine and multiple osteolytic lesions led to the diagnosis of lambda light chain multiple myeloma. Synovium, subcutanous nodule, kidney and bone marrow biopsies revealed amyloid depositions. An operation material for carpal tunnel syndrome revealed positive staining with lambda light chain antibodies. There were no cardiac, hepatic and glossial involvement. She responded well to treatment with cyclic oral melphalan and prednisone. Articular manifestations improved, bone marrow findings were decreased and serum protein abnormalities electrophoretically regressed and renal functions returned to normal values. We discussed infrequent amyloid joint disease in a patient with lambda light chain myeloma.
