THE IMPORTANCE OF PATHOLOGIC COMPLETE RESPONSE IN RECTAL CANCER PATIENTS TREATED BY PREOPERATIVE CHEMORADIOTHERAPY

The prognostic value of pathologic complete response (pCR) to preoperative chemoradiotherapy in rectal cancer patients was aimed to be evaluated. Sixty-five patients with stage II-III rectal cancer treated by preoperative chemoradiation followed by radical resection were evaluated. Recurrence and survival were compared with tumor characteristics and pCR. Tumor downstaging occured in 77%, including 20% who had a pCR. None of the clinical or pathologic variables were associated with pCR. The estimated 5-year rate of local recurrence was 14%, of distant metastasis, 26%. Only one of the patients with pCR has developed disease recurrence (distant metastasis). A significant difference was found in disease-free survival among patients with pathologic stages I, II or III tumors, but not in overall survival. A pCR to preoperative chemoradiation is found to be associated with improved local control and patient survival.
