Transfusion of blood from one person to another is mostly a problem of immunity and infection. The aim of the present study was to examine cost-effectiveness and transfusion reaction differences between the autologous and homologous blood transfusion, in patients who were scheduled for an elective surgical procedure. Methods: The study included 37 patients who underwent an elective general surgical procedure at Social Security Foundation Ankara Education Hospital during 1995-1996. The patients were divided demographically into two similar groups who received autologous (n-17) or homologous (n-20) blood transfusions and compared the results for transfusion reactions and cost-effectiveness. Results: The analysis of the results showed autologous blood transfusions, prepared from the same person a week before the elective surgical procedure, caused decreased transfusion reactions and lower costs compared to the homologue transfusion group. Conclusion: This study and previous observational studies support the hypothesis that peri-operative homologous blood transfusion are associated with increased transfusions reactions and increased costs when compared with the autologous blood transfusions. Pre-operative autologous donation of blood is a safe and cost-effective procedure for patients who undergo an elective surgical procedure.
